While hospice care is routinely provided in the home, circumstances sometimes require that care be delivered in an alternate setting. For this reason, hospice care may also be provided in the following locations:

    • Acute care hospitals
    • Nursing homes
    • Assisted living facilities
    • Group/adult homes
    • Mountain Valley Hospice House

Occasionally short-term inpatient hospital care may be required for pain or symptom management which cannot be controlled in the home setting. When this is necessary, Mountain Valley Hospice may admit patients to one of the following contracted hospitals:

    • Little Falls Hospital
    • Nathan Littauer Hospital
    • Saint Mary’s Hospital
    • Saratoga Hospital

Individuals residing in nursing homes, adult homes, group homes, and assisted living facilities have a right to access hospice services. The expertise and services offered is also provided to patients living in these facilities.

    • Fulton County Residential Health Care Facility
    • Lexington ARC
    • Liberty ARC
    • Nathan Littauer Nursing Home
    • Palatine Nursing Home
    • Pineview Commons
    • St. Johnsville Nursing Home
    • Wells Nursing Home

In 2005, Mountain Valley Hospice opened a hospice residence— a home for individuals requiring all stages of care from assisted living through total care. This home, honoring the memory of the founding executive director, Nancy Dowd, is located in a rustic setting within the city of Gloversville. Detailed information about The Nancy Dowd Hospice House is available at this link.
