What is Hospice?

    Hospice is a special kind of caring for terminally-ill persons and their families. Hospice is dedicated to helping patients and their loved ones enjoy the best possible quality of life each day.
    Hospice care focuses on providing comfort by relieving pain and other physical symptoms, as well as attending to emotional, spiritual, social and economic needs which arise during the final stages of life.

What is Mountain Valley Hospice?

    Mountain Valley Hospice is a community-based, non-profit home care agency founded in 1988. It is Medicare certified and licensed by the New York State Department of Health. It is a member of the National Hospice Organization and the New York State Hospice Association.

Who Provides Hospice Care?

    Hospice patients receive comprehensive care under the direction of the Interdisciplinary Care Team. Services provided are:

  • regular visits by a Hospice case manager.
  • personal care assistance by a home health aide.
  • care coordinated with the patient's physician.
  • Hospice medical director consultation.
  • a full range of counseling services by a social worker.
  • spiritual counseling, if desired, by the Hospice chaplain.
  • medical equipment and medications as needed.
  • assistance and companionship by Hospice volunteers.
  • nutritional counseling.
  • bereavement support for family members for one year following the patient's death.
  • professional staff on call 24 hours a day.
  • an individual plan of care.

Who is Eligible for Hospice Care?

    Mountain Valley Hospice offers care to residents in Fulton County, western Montgomery County, southern Hamilton County and northwest Saratoga County who have an advanced illness and have a desire for supportive care and comfort measures. Mountain Valley Hospice serves terminally-ill patients regardless of race, age, gender, creed, or financial status.

How are Referrals Made?

    Referrals may be made by anyone familiar with the patient, including family members, physicians, hospital discharge planners, clergy and social service agencies. Referrals made be initiated by contacting the office of Mountain Valley Hospice at (518) 725-4545.

Who Pays for Care?

    Admission to hospice care is based on need and hospice services are not denied because of inability to pay. Hospice expenses are covered by most major health insurance plans including Medicare. In cases where insurance does not pay for all services or an individual is uninsured, care will not be denied.

Community Support?

    Mountain Valley Hospice is a non-profit organization which relies on community support to assure that all persons who need and desire hospice care will receive it. Donations received as memorial gifts, contributions, grants and charitable support are gratefully accepted and acknowledged. All contributions are tax deductible to the extent allowed by law.

We'll have to see what needs doing.

Maybe I'll wash the dishes,
or tell a joke, or bring you some soup,   or play cards with Freddie.

If you want to talk, I can listen.
If you don't want to talk, maybe I can answer the telephone.

If tempers are flying, maybe I can soothe them.

If you're not sure you're doing what's best, maybe I can tell you you're doing just fine.

If you have to get some rest,   Maybe you'll trust me to sit by her bed.

If the commotion's getting to you,    maybe I can steer you in-laws out to dinner.

If you're angry, maybe I can say that's OK.

If you can't bring yourself to tell her goodbye, maybe I can make it easier by saying it first.

If your strength is failing, maybe I can reach out my hand and keep you going.

If you're falling apart, maybe I can put my arms around you and hold you together.

If you're feeling helpless, maybe we can pray together.

We'll have to see what needs doing.

Maybe you won't need me for anything!

Maybe it'll happen so quickly or you'll have so much support you won't even need me in your house.
