What is hospice?
Hospice is specialized healthcare focusing on pain and symptom management of progressive and incurable illnesses. Care focuses on the aggressive treatment of the illnesses’ effects when curative treatment of the disease is improbable.
What is Mountain Valley Hospice?
Mountain Valley Hospice is a community-based, not-for-profit home care agency founded in 1988. It is Medicare certified and licensed by the New York State Department of Health. It is a member of the National Hospice Organization and the New York State Hospice Association.
What does hospice care include?
Hospice patients receive comprehensive care under the direction of the Interdisciplinary Care Team.
Hospice care includes:
• Regular visits by a hospice case manager.
• Personal care assistance by a home health aide
• Care coordinated with the patient’s physician
• Hospice medical director consultation
• A full range of counseling services by a social worker
• Spiritual counseling, if desired, by the hospice chaplain
• Assistance and companionship by hospice volunteers
• Medical equipment and medications as needed
• Nutritional counseling
• Bereavement support
• Professional staff on call 24 hours a day
• An individual plan of care
Who pays for hospice?
Medicare, Medicaid and most commercial insurances fully cover hospice care. Importantly, Mountain Valley Hospice provides care regardless of an individual’s
ability to pay.
What is the cost of the hospice house?
The room and board rate for the hospice house is $250 per day.
How are referrals made?
Although a physician is required to admit someone into hospice services, referrals may be made by anyone familiar with the individual, including family members, friends, healthcare providers, social service agencies or individuals themselves.
