The
Referral Process
When patients are referred
for Hospice care within only a few days of their death, it is very
difficult, if not impossible, to begin to work with them and their
families.
May we suggest that the
patient and the family be referred as early as possible so we may serve
them to the best of our ability and for their utmost well-being.
Referrals may be made by
anyone familiar with the patient, including family members, physicians,
hospital discharge planners, clergy and social service agencies.
Referrals
may be initiated by contacting the office of Mountain Valley Hospice at
(518) 725-4545 and asking for the Patient Care Coordinator.
If the referral is made by
anyone other than the physician, Hospice will consult with the physician
prior to visiting the patient/family. After the physician makes a Hospice
referral, the Patient Care Coordinator will call the patient/family to
schedule an information visit to explain the Hospice
philosophy, palliative care, services, etc... The physician will be
notified after the information visit is made, with the outcome and plan.

Valley Hospice Admission Criteria:

Any advanced illness

Supportive, comfort
oriented care

Physician who will
certify for Hospice care

Resides in geographic
service area as defined by New York State Department of Health.
