Where We Provide Care

While hospice care is routinely provided in your home, circumstances sometimes require that care be delivered in an alternate setting. For this reason, hospice care may also be provided in the following locations:

  • Your home
  • Acute care hospitals
  • Nursing homes
  • Assisted living facilities
  • Group/adult homes
  • Mountain Valley Hospice House

Occasionally short-term inpatient hospital care may be required for pain or symptom management which cannot be controlled in the home setting. When this is necessary, Mountain Valley Hospice may admit patients to one of the following contracted hospitals:

  • Nathan Littauer Hospital
  • St Mary’s Hospital

Care can also be provided in Nursing Homes:

  • Nathan Littauer Nursing Home
  • Palatine Nursing Home
  • St. Johnsville Nursing Home
  • Wells Nursing Home

Care can be provided in places that others call home:

  • Lexington ARC
  • Liberty ARC

Care can be provided in Assisted Living Facilities:

  • Willing Helpers Home
  • Pineview Commons

Mountain Valley Hospice House

In 2005, Mountain Valley Hospice opened a hospice residence— a home for individuals requiring all stages of care from assisted living through total care. This home, honoring the memory of the founding executive director, Nancy Dowd, is located in a rustic setting within the city of Gloversville.
